Enrollment forms must be completed and turned in to the Enrollment Office for all new students entering the Monroe School District. The Enrollment Office is located at the District Administrative Center, 925 16th Ave; Suite 3, Monroe, WI 53566. Office hours are Monday - Thursday, 8:00 a.m. - 4:30 p.m. and Friday 8:00 a.m. - 4:00 p.m. Please note, you will need to provide the child's birth certificate, a parent/guardian picture ID and proof of residency to enroll a student. The fee schedule is based on each building's requirements for fees.

**Registration Starts January 9, 2023.
The School District of Monroe will be accepting online enrollments for the 2023-2024 school year starting January 9, 2023. For those who can’t access the online platform, please visit the District office located at 925 16th Ave, Suite 3 to enroll in person. Children who are four years of age on or before September 1, 2023 and reside within the School District of Monroe, or wish to open enroll, are eligible. Site preference is granted on a first-come, first-serve basis.
To protect students from diseases that are preventable by immunization, Wisconsin law requires all students to show that they have received the required immunizations or have a signed waiver on file.
Any parent/guardian wanting to request a building or transfer must complete the Building Transfer/Request Form. Requests will be reviewed and renewed annually based on class/grade level sizes not exceeding District guidelines. Completed forms should be turned in to the Enrollment Office.
An alternative bus form is required for any student being transported to a location other than their home. The form is required to be submitted every year. Completed forms should be returned to Lamers Bus Lines, 2665 3rd St. North, Monroe, WI 53566 or by fax to (608)325-7767.
A Disclosure Statement is required every year for classroom and field trip volunteers for approval prior to working with students. Completed Disclosure Statements can be turned in to any school office.
